I can start it off with what I'm sure is a very incomplete list.
San Bernardino Co 911: AMR, Desert Ambulance in Barstow, Baker EMS in Baker/Needles, Symons Ambulance in Bishop
LA Co 911: McCormick in Malibu/Calabasas Carson and Southbay, AMR in Antelope Valley, CARE in SGV and unincorporated South LA down to OC, Cole-Schafer in SGV, Gerber in Torrance, Americare in Santa Monica.
In terms of LA IFTs, there's a million and one and most aren't worth your time.
The decent ones: Bowers/Pacific, PRN, Medresponse, Impulse, Americare, Ambuserve, Liberty, American Professional, Royalty, and I'm sure I missed a few more non Medicare fraud companies.
My advice would be to try briefly to get on with AMR and CARE since they're always hiring, realize you will push a gurney for LACoFD, and get out to San Bernaghetto, or even better Kern Co.

Los Angeles County 911 Ambulance Exclusive Operating Areas 2006-2016

http://ems.dhs.lacounty.gov/GIS/Maps/Ambulance061907a.pdf

Note it only shows the County EOAs. All the white areas on the map, local FD ambulances handle the call. Exceptions are Gerber in Torrance, and AmeriCare in Santa Monica.

Note that Westmed is the parent company of McCormick so even though the map says Westmed, it's really McCormick responding in those two EOAs.

(I'm just curious, has anyone managed to find a similar map for OC?)
