As far as I know, JCAHO does apply to hospital based EMS because hospital based EMS is considered an extension of the hospital.
JCAHO does not apply to private EMS because they are not hospitals. However they can get CAMTS (Commission on Accreditation of Medical Transport Services) certified. And I believe there is another one or two, but I forget the name(s).

You will not be required to do all of the JCAHO mandates and most will specific to your job title. As a hospital employee there will still be some things that much be done such as whatever requirements your job description says such as CPR, ACLS, PALS as well as a few general patient safety courses.
Vent is correct. I however think that looking over the Joint Commission's (what they are officially known as again) National Patient Safety Goals and standards and practicing them as best you can will improve the quality of care at the EMS level.
However this is important to remember. As a hospital employee that can deal with patients, if a tracer is performed and it goes to EMS they can pull your file (look for certs) and they may ask questions that you need to know the answer to (like the process of effective hand-offs when going straight to a floor and such). If you are preparing for a survey best bet is to call up quality management and they will tell you everything you need to know.

JCHAO didn't seem that bad this last few weeks. We had them on my floor while they were inspecting. Then again, and this is why I love my RN Manager, our floor is run like JCHAO are there every single day. Makes for a very very efficient floor.
And one of the other ambulance accredidations is CAAS. That's what the 911 private company in ABQ is.
