Looking for Advice, Options, Solutions:

I was an EMT-B in the State of Pa. Long term options were up in the air. Took a bridge course to I/85 Web based with one week skills and passed in Idaho...flew out there. Did the I/85 to AEMT Bridge, Online, 1 day skills, flew to Georgia. Coming up on 3/31/15 expiration date. While that is several weeks away, to me I am attacking as if it was next week.

Pa. was suppose to start with I/85's in 2013, never happened. AEMT was just added to scope and I was told to apply under reciprocity. No current Protocols for AEMT, scope seems to be c/w National Standards. Meds and need for Command are unknown. Protocol release date is later Jan 2015, Protocols take effect in June / July. Pa. has a unique way of doing things Commonwealth wise. Legislation was passed recently to bring Narcan to BLS and Police, you all know the trend for that. AEMT appears to have been a part of the bill.

Right now Pa. has EMR, EMT, Paramedic, so I have no affiliation with an Agency. AEMT Refresher in progress, CME is okay, have maxed out on online CME. Would appear an ACLS, PHTLS, and or PALS refresher would fill the void. ACLS and PALS via 100% online. Not sure where that applies with the online CME.

I was in a Paramedic Class and okay academically, unable to maintain clinical hours, long story, another story for another time. I completed a little over 1 semester. All AEMT skills were covered in the class with skill sign offs to pass finals for the semester. Pharmacology meds for the National Scope were covered and passed.

My problems are Officially outside of clinicalls while in Medic school, field and ER, I have not used AEMT Skills as an AEMT for, we do not exist. I need a Medical Director to validate my package. One would assume skills would be a part of that process. With no firm date of a License for the State and inquires deflected, I am left in a situation of needing skills signed off within a State that as of today, does not have AEMTs.

Affiliation, very grey area. I am a Volunteer not working as a paid EMT. I am a Vol FF and EMS Line Officer but due to the rural area and $$$, all we run EMS wise is a QRS. I have a local Doctor who serves as a Medical Director with respect to Epi-pens for EMT use. Getting him on board with AEMT with out precedent, seems like a vain impossible option. I have considered trying to talk him into it but that would mean liability issues, fraud, and if there were an audit...doom.

Options I have come up with, that I am chasing.

Paramedic Skills review through State EMS Con-Ed with the hope I can be evaluated on AEMT Scope Skills. Maybe the Instructor signs off but can a Medical Director sign of based on this option with no AEMT Protocols in place?

Hands on ACLS would hit some of the National Scope...PALS Hands on would hit the other. Again, can that translate into skills sign of by Medical Director, with no AEMT Protocols?

The Pa. Bulletin that establishes the AEMT Scope is published minus specific meds, the way it is worded refers to a previous Bulletin of ALS Drugs a Paramedic can use.

NREMT allows In-active status for AEMT but stipulates first time AEMTs must have 6 months of field time to change to that status. NRAEMT Status is needed to obtain PA AEMT, whenever that happens. Have reached out to Regional Medical Director, have not heard back. Just so happens he is an Instructor with the Medic class I was in, so he knows the inner workings of the class. Have not heard back and while I have followed the chain of command for the most part, reaching out to him is circumventing the chain of command. Seemed logical as to the best of my knowledge I am one of a hand full State wide seeking Reciprocity but an unusual situation in what is the beginning of a new status State wide.

So has anyone, Instructors or what not, comes across a situation like mine and does anyone of advise?

Call NREMT they are going to be the only ones who can give you a definitive answer. But from what you're saying it sounds like you got certified at a level that isn't recognized in your state yet. You may be out of luck and have to revert to EMT for national registry.
