1) this was an ambulance that rolled over after being struck by a car
2) the person was facing forward, with no patient; absolutely no reason to not be seat belted
3) I'd be lying if I said I always wore a seatbelt in the back of the ambulance; in the front, almost 99% of the time, but in the back with a patient, not often.

that being said, culture has changed since I started (more stay and play on sick patients, no more CPR in a moving ambulance, etc), more focus on securing all equipment to make it crashworthy, so I do think that if you can do your job while seat belted, you absolutely should. and the patient should always be properly secured, with shoulder straps.
