DL51A Confusion

Just Got my EMT-B certs in Los Angeles and trying to get a job. Most companies seem to require a DL51 and card to apply. So I printed the DL51 from the DMV website, went to my Doctor and filled out the form, she didn't have the card. Went to the DMV, they didn't have the card. Does your employer supply the card? and if so, how am I supposed to have this before I am even employed?

It's a little green card that is supposed to be attached to the DL51 form. The DMV should have them.

This - they are stapled to the medical form at my local DMV.

Thanks, apparently the lady I talked to at the DMV gave me bad info. I'll go back tomorrow, hopefully with better luck.
