BLS Bag Inventory Control

Greetings; medical calls are not the primary function where I work, but they do happen occasionally. Anyways, we have a basic BLS medic bag as shown in attached pdf. What I need to do is put together an inventory list of bag contents and have a place for our guys to check off what has been used up. I also need a place in the log to record customer site where materials were used. This is for billing purposes, our customers are company industrial sites.
So... I was wondering if anyone had a sample form I might be able to use as a template to create one from. Our customers (who pays the bill) are companies and not individuals, so we don't need insurance info and what not.

I might still have a list of available for you to use, but, I'm going to have to check my records to see if I still have it. It's been quite a while, I don't know if I still do.
 
Just make a daily check-off sheet on Excell or something and print it off. Have the date and site on it, and make the person responsible for the bag that day sign it. Restock at the end of the day.

Something with only a few columns like this would work, I imagine:

Item - Quantity - Needed - Location Used
4x4s - 20 - 0 - n/a

If you use a 4x4, it'd read:
4x4s - 20 - 1 - site 3

For an ambulance crew I can see something like a daily inventory being needed, but in our situation that is not the case. We pay our guys a good salary for a certain function and taking inventory is not one of them.

Like I said, the BLS bag is seldom used and is kept stocked. The only time the bag contents should change is when used and in that case, we need record of it in order to replenish the bag.

Maybe it's just me, but it seems as though you're over thinking this. It appears that all you need is a pre-printed sheet listing the contents of the kit - a basic inventory sheet - and next to each item a space in which a number can be entered representing how many of an item has been used. Add a space at the top to enter the customer name/location/whatever, place the sheet in the bag and there you have it: a simple form that even someone who gets a good salary for not taking inventory can use. ;)
