Ambulance Drivers License- double checking

I just wanted to double check on here that this is all I need to receive an ambulance drivers license in California:
Medical Examiners Paperwork filled out
Live Scan fingerprints done, with the paperwork filled out
Pass the written test
Pay the fee

I believe that is all. Make sure the person who did your medical exam is authorized by the DMV to do the exam.
