What is the best way to place ecg leads without actually palpating the intercostal spaces? No one seems to ever do that.

If we're being perfectly honest? Look to where you'd put the heel of your hand for chest compressions, slightly up from that and on either side of the sternum is v1/v2, v4 goes 1-2" below the left nipple, v6 goes mid-axillary in line with v4, v3 goes in between v2/v4, v5 goes between v4/v6.

This is just one of those things where as you gain experience doing the procedure, you just start noticing where you place the electrodes. You see where they’re placed and you notice the landmarks… Eventually you get to the point where you can place those electrodes reasonably precisely because you just know where those electrodes need to be. You learn how to properly adjust for differences in a patient’s size and you’ll just do it without realizing you’re doing it.
