Up north in New Brunswick we own our license and scope of practice. We are considered medics 24/7. We do not work under a MD. To license it is 550 a year which includes dues to our college of paramedics (paramedic association) and liability/malpractice insurance.
When working for a company that has a medical director he signs off on extra skills if any and the protocols used are Provincial, set by the department of health. We have medical control for deviation and advice if needed as well.
When we are off the clock or are with a company without a medical director (volly fd for example) then we still are able/required to practice to the extent of our abilities and scope of practice.
Are there any states or other places that are like this? I know there is another province or two here in Canada that is similar though I am not sure to what extent.

I gather Australia may be closer to you than to USA in that regard.

Being a provincial "Amurrican", that sounds unnatural.;)
