I messed up and applied online at NREMT. They never mailed my application, and I was out of time. I was gone for a year doing my tour of duty. Well, I messed up the form and have to mail it in messed up. Then they will mail it back to me and tell me to fix it. I want another application to start working on so I can run around and get signatures. Apparently, the option of downloading the rereg form is not available. I scoured the net looking for the application. Does it exist? I need one for EMT-B. I hope someone can help me get the application for my rereg. :o
 
Go to your local EMS office... they have stacks of them in the office. If you dont see it, ask someone they will gladly give you one.
 
Check your states EMS Bureau's website. Ours has all the applications/forms in PDF format.
