Except for my family situation, I would work and be happy in this system. This is a pretty high-quality EMS system, with high-level QA, fairly progressive protocols, and long transport times (which everybody knows is where the fun is).

Some other information (not included in job posting)

The ALS spots that are currently open are 24 hour shifts, working the Fairfax schedule. The BLS spot is a 48 hour per week position, and the schedule is yet to be decided. You might work 4 12's, you may work something else. I don't know. There is some opportunity to move onto a 24 hour shift from a 48 hour workweek, but you have a better chance of that if you are about to be or planning to be an ALS provider.

We do not have a rookie school, more like an orientation. You need to have all of your training prior to applying. If you aren't sure if your fire stuff transfers over, call them and ask. I have no idea.

Most of what we do is EMS, but we are all cross-trained and carry our fire gear on the ambulance. There are also opportunities to become engine drivers and be assigned to a station with a more fire-oriented approach to things, but remember, this is a rural county and EMS is still the main function, with some fire thrown in here and there.

We are a small system, staffing 4 ambulances at a minimum a day to cover a 343 square mile area with a population of around 34,000. Transport times range from 20 minutes to 45-50 minutes, and rarely up to an hour or more. I feel like we see plenty of sick people, and our call volume is approx. 5500-6000 calls a year.

The equipment is better than most. We have lots of toys, such as Autopulses, Ventilators, new LP15's with CO monitoring, etc, and the ambulances are also decent. I really don't know much more to say about that.

I feel like where we shine is our protocols, which are exclusive to Orange County. While we are minimally associated with the regional EMS councils, we write our own guidelines. A few highlights are: Using the Canadian C-Spine Rules to clear C-spine in the field; OG tubes/NG tubes; A very aggressive pain-management/sedation guideline where we have the freedom to treat anything we really want; RSI with no age criteria; Induced hypothermia; and we just began prehospital lactate testing.

We have a 100% QA/QI program, which means every call we go on gets looked at somewhere. They are all evaluated using a guideline that makes sure all billing requirements are met, and appropriate care was given. We stress that our protocols are "guidelines" and alot of treatment is based on clinical judgement.

It's alot of stuff, and all places have good and bad things, but if you are looking for a progressive, growing department with long transport times and decent equipment/protocols, then check it out.
