DL51 Question

Hello! I'm applying for an EMT job in Southern California and am working on getting my DL-51 form done. I was talking to my doctor and she wanted to know whether the physical exam needed for this form is a "DOT physical" or a "pre-employment physical"...I wasn't even aware there was a difference, but does anyone know which one it is? Thanks, any help would be greatly appreciated!

To the best of my knowledge, it's a DOT physical and needs to have the components listed as well as the clear/don't clear/clear with conditions listed.
