Are crew member (EMT/Medic) signatures required on the PCR? Full names? State certification numbers?

Yes, that information should be on the PCR. If you're using an electronic charting system, you can electronically sign a chart, and your credentials & certification numbers should be printing on the chart.

In my own personal experience, the bulk of insurance denials are not cost saving measures by the insurance company...it is actually the result of the doctor not documenting properly and explaining the need or the thought process behind the request. This is a team sport...your doc needs to slow down, compose proper and legible charts and ensure his/her thought pattern is clearly communicated.

Often, once my billing team gets the denial they are then able to provide further justification based on what they know from our internal procedures or they will chase the provider down and get more information to get the claim approved.
