So how does ALS work in California?

Just a question: do we ever intubate breathing patients? Wasnt given clear instructions on if the pt is breathing vs not breathing. As in breathing, but wont support their airway much longer, or barely breathing, etc.

What is your national drug guide you've use in school?

It seems to me that every school uses a different "national" drug guide, with different drug dosages (with them more or less being the same) and it makes me wonder that shouldn't we all have the same knowledge and information regarding our paramedic education on a national level? In my school...

    Common Meds Made Into Drips

Just a side question: Any drug with a long dosage time (like Mag Sulfate for example) can only be given via drip? Like push rate for Mag is 5 to 20-60 minutes depend on use, but we cant do very very very slow IV push?

When working in a smelly situation

You know, foul-smelling, is it offensive to wear a mask or get something to cover up your nose? Serious question. I mean, I've smelled some pretty gnarly stuff before, I'm sure we all have, and I'm not talking about usual odors and stenches, I'm talking about the 5% of smells we hope to never...

If you could work at any EMS company or agency in the U.S. ...

A little elaboration on "horrible protocols" ? I never understood this idea. I thought protocols were more or less constant throughout the country, thats why we have our national scope, so different states and counties can vary or modify as they please. But anyways, on horrible protocols. Is it...

    Trauma activation question

So I know different counties have major trauma criteria, and from my knowledge, a trauma is any physical injury that the body didn't do to itself, maybe a fall. This includes blunt force, penetrating, fractures, etc. But from my protocols it says that if we get any one of the criteria from...

    Right sided EKG/Nitro

Are you given any protocols on the use of a Right sided EKG and/or the use of Nitro with it? I was taught to do a right sided EKG when you have an inferior wall STEMI, and you would switch v4 to the right side, but my question is what would you expect to see? Would you see STEMI in v4, as well...

    Does sub-q Epi work?

I've heard stories of people using sub-q epi, but hoe does it work? Doesn't your skin and directly underneath it have really poor circulation, and that's why people use IV epi, because it has the best circulation and drug response. Or when people are in shock theres not any good perfusion...

    Seizures and 12 lead

I heard of a story of a medic intern doing a 12-lead on someone c/c of a headache only. 12-lead showed STEMI. Perhaps they were really lucky to have caught that? But do your guy's protocols say do a 12-lead with an active seizure? Or postictal ?

Penetrating traumatic arrest, work or not?

Hi guys, just the protocols I have in my county that I'm learning from states: Trauma arrest - Adults Blunt arrest/ penetrating arrest/ prolonged extrication. That's all they stated on trauma arrest. Just to clarify, that mean's NO intervention right?

    Sodium Bicarb

Hi guys, just got some questions on Sodium bicarbonate. So I know this is used to treat hyperkalemia and acidosis, but the thing is: you know you're going to get peaked t-waves and widen qrs with hyperkalemia, but how do you know the s/s of acidosis? Granted a history would help, but is that...

    IFT paramedics

Also, lets say you go on a scene call. And you determine that the pt is unstable. Do you load and go, or call for "911" ALS? My logic is that they can do everything you do, and vice versa, just a matter of having the same equipment. On the opposite hand, do hospitals get mad if they have a pt...

Penetrating traumatic arrest, work or not?

It's also protocol based, like in my county. Whether or not its outdated thinking, we still gotta do everything right, just a matter of agreeing with it or not.

Penetrating traumatic arrest, work or not?

From my knowledge we dont work traumatic arrests because they have a hole somewhere that we can't fix. However, pediatrics we always work them up and transport. Anyone know the reasoning for this?
