Southeast Texas (Houston Metro) Jobs available

From what I understand their supervisor vehicles are stocked with plasma and blood and they used it within the first 6 hours of implementation. Being in Houston I'd assume their bank is the Gulf Coast Regional Blood Center. Not too familiar with transport times but being in north harris county...

LJEMS has the potential to be an amazing service, not just one that seems good from the outside. lol

    Chest tubes

Ah you're right that's in their current guidelines, just saw the ones above are from 2012. They can do a simple thoracostomy and/or pericardiocentesis on their traumatic arrests.

    Chest tubes

Unless I'm missing it MCHD doesn't have a chest tube protocol, only pericardiocentesis.

    Scope of Practice

Medic/RN. We have the exact same scope and can do and give whatever our medical director allows (yay Texas)! We're in the process of getting blood, usually no IABP but do an occasional impella transfer.

    Flight Medic Questions

From what I can remember Parkland, drip rates, gas laws, pedi drug dosages, etc... Didn't have any invasive hemodynamics or ABGs, but I might have just lucked out.

DO you video? Emergence of video assisted laryngoscopy in the field.

Which is exactly what this tread is about, personal preference for the different devices. I haven't seen anyone say one device is the absolute best, they all have instances where they are the better choice over the others for intubation due to variations in design. Information like that is not...

DO you video? Emergence of video assisted laryngoscopy in the field.

We just got the Intubrite VL at my flight job and I use the McGrath and King Vision at my PRN jobs. My personal preference is McGrath > Intubrite > King Vision. The McGrath beats both in image quality, compactness, and ease for initial DL use, however the biggest drawback (for some) is the lack...

    Nitro patches for strokes

Depends on who you ask. It's not being pushed quite as hard as it was say 5 years ago but it's still widely considered the standard of care. AHA gives it a Class I level A recommendation, ACEP has it at a level B. In Houston they have a specialized ambulance that responds to suspected strokes...

    Nitro patches for strokes

Yes, obviously tPA isn't used in bleeds. There is a tPA screening checklist that is widely used that will guide the ED physician as to whether the patient is eligible to receive treatment, with one of the exclusions being "Elevated blood pressure (systolic >185 mm Hg or diastolic >110 mm Hg)"...

    Nitro patches for strokes

Typically the consensus is to only treat if the SBP > 220 and/or DBP >120 or if the patient is tPA eligible for which they need to be below 185/120.

    Narcan Use Survey

Not sure why it would be so hard to remake the survey to be less biased towards the answer you're looking for. Anyways, I've never used it for a diagnostic tool and it shouldn't be used as one.

    Harris county emergency corps

It's in the guidelines but not in practice yet. Ned consult is a phone to either the medical director or one of two medical officers that are experienced Paramedics, just depends on who answers the phone. Currently you only need to call for Ketamine or RSI if a Supervisor isn't available...

    Harris county emergency corps

There isn't a set time but you'll just have to wait until your in-charge partner, supervisor and the clinical department are confident in your abilities. Don't expect it in less than 6 months of hire. HCEC is pretty off line driven other than Ketamine usage and RSI which you can do if a...

    Harris county emergency corps

Station quality: Very high compared to most services around. Call volume: around 25k a year. Typically 8-12 calls a shift. Pay: I believe medics start around $16 with no experience Overall: It's a great company with an involved medical director. You can typically advance to in-charge within...

    Critique this RSI protocol.

Yes, that is the (main) reason for RSI. It's easier to stabilize and secure a patients airway before they stop breathing on their own.

    Critique this RSI protocol.

I'm also not a fan of that protocol. I typically use Fentanyl 3 mcg/kg if pre-treatment is indicated, Ketamine 2 mg/kg for induction (Etomidate if patient is hypertensive) then 1.5 mg/kg of succinylcholine.
