I had searched (& found) related info on here, but i just applied & am in the process, so i wanted to pass along what i found out / experienced. First off, take defensive driving & get your proof; i took a comedy DD course & got my cert in Irving the next day. Then get a form (2 pages, actually) from your employer. Management signs, you sign, notary signs. Take this form, your state cert, DD cert, $40, and drivers license down to 3131 Dawson Ave in downtown Dallas; it's a bland brown building right next to a bunch of other ones - with EMS vehicles nearby. Go upstairs, submit your info, pay $40 (cash), then wait 5 business days (this is where i am) - after that, you call a number, verify passing background check, go back downtown, take your pic, & yer good to go.
