They ask you.
If they take you to a hospital, the EMT's can often get mre info from the hospital folks to fill out their mandatory field paperwork...which has enough info to start the billing process.
An experienced case manager or billing technician (clerK) will screen for and spot incionsistencies in stories to get the gelt. And if all else fails, you are held in a deadbeat file and/or turned over to a collection agency, and they get serious (gop to zabbasearch).

We generally ask the patient. Also, billing information is able to be shared between health care providers.
