EMS Response to the Coronavirus outbreak

The only N95 our company has available to purchase are the cheapest ones you can find. I have my own that I will use if I ever have an actual concern.

    Resp failure ift pt

When talking about NPPV it is important to be specific. The first sentence says in the ER the patient was placed on BiPAP at 7, which is probably the EPAP. Were they on CPAP the whole time or started on BiPAP in the ER then transported on CPAP? Did they get an ABG? There is a lot working...

How did you get an ETC02? Probe between the mask and tubing or a ETC02 NC under the mask? I have found the latter to be more reliable depending on vented vs non-vented mask and leak. Would lowering her RR be helpful for her acidosis? Unfortunately only so much you can do with a pneumatic...

    Paramedic school vs Nursing school

Yeah it is really awesome to be the hero who intubates grandma and saves her life so she can die a week later from VAP/CAUTI because you are too worthless to realize how important basic nursing care is. Solid work. If you want to see some of the biggest impacts on modern medicine in regards to...

Montana Firefighter Allegedly Fired Over Instagram Workout Photos

Sounds like this was clearly a case of discrimination and not having a prior social media policy was huge oversight. However... "Prior to her termination, the department issued Pritchard verbal and then written warnings that she had utilized taxpayer resources for her own benefit, posted a...

    3YO Male, Sick

Immediate? Nothing You are advocating to cardiovert this kid with a 25 min ETA to a Pediatric Hospital? This is not something that needs to be treated in the back of an Ambulance without further diagnostics

    3YO Male, Sick

I really do not think it is wise to go down that road with this kid. Unless you are hours from a hospital and he starts deteriorating this is best left for a Pediatric Intensivist or Cardiologist to decided. There are clearly multiple issues going on and SVT probably isn't the primary.

WHATS THE PATH TO AIR MEDIC

Appreciate the enthusiasm for the military however it is just not comparable to civilian HEMS. AE is primarly transport of stable patients with little to no interventions. AE Techs are not trained at the medic level. PJs are technical rescue experts and solid medical providers at the paramedic...

    3YO Male, Sick

Not sure it was necessarily questioning your knowledge however, outside of the pediatric CTICU, saying SVT in this scenario is unequivocally a cardiac emergency is a little bit of a hyperbolie. Not all SVT is some channelopathy or structural heart disease nor likely will lead to irrevocable...

Assuming pediatric would be a Tibial or Distal Femur IO so the flow rate and time to central circulation isn't all that great compared to an EJ PIV which means medications like Adenosine may be more likely to be effective with the latter? Just a guess

    Stethoscope - The Mega Thread

The Cardiology series are well worth the investment in my opinion. I have both the Master and IV. The IV is a fantastic stethoscope but for EMS work I prefer the Master. The dual head on the IV has it's advantages however I never really utilized it and it is fairly bulky, occasionally gets...

To play Devil's Advocate, most people who say "nursing ain't all that great" are ignorant of nursing beyond what they see at the bedside. Are you an 18D? Been to SOCM? Because if you talk to most SOF medics you'd know that it isn't "PA level" stuff that keeps your patients alive in austere...

Is a Portable Ultrasound the future for EMS

To play devils adovcate I doubt any trauma center will be taking patients directly to the OR based off EMS FAST exams. It will assuredly be redone in the ER before. Even progressive HEMS services who do POCUS frequently are verified by a physician on arrival. Obviously having a pre-hospital...

Is a Portable Ultrasound the future for EMS

I have a fascination with POCUS and would love to have it on the helicopter but having said that I think it has limited utility in most EMS environments and the required education and skill to obtain and accurately interpret anything other than blatantly obvious findings is more than most...
