    And the jury is back

We’ll agree to disagree I suppose. Working with the police with agitated patients is a not insignificant part of EMS call volume these days.

I guess I don’t get it. No medical care should provided until arrival at the hospital?

My AMR operation gave up on getting crews to lock the doors with the keys (only have one, non remote key). They installed these and they work great. https://tremcopoliceproducts.com/anti-theft-system/

    And the jury is back

I’m not sure I agree with making it a law enforcement problem. If there is an underlying medical or toxicological issue, it would seem the individual is now a patient and in need of care. If a provider feels that a patient in custody needs immediate medical intervention, that needs to happen...

    And the jury is back

Much in the same of the Vanderbilt case, it was not the medication administration that led to the catastrophic outcome, but what happened next. Both a moderate overdose of Ketamine and inadvertent use of vec do not have to be fatal if the patient is managed properly. The poor monitoring and...

Paramedics Found Guilty in Last Trial in Elijah McClain Death

https://www.nytimes.com/2023/12/22/us/elijah-mcclain-death-trial-verdict.html Lots to unpack. Was it necessary to sedate this person? Was the dose dangerous? Are criminal charges appropriate? We could go on. Whatever happened, to me this boils down to failing to monitor the patient. Sedating...

Nothing too dramatic, just started running rough at post and died. Another crew got hit transporting an hour before (no injuries), busy day for the tow company. Good riddance to this trashed transit I hope, the other crew was in a brand new type III :(.

    ASHI

We heard they were not supporting a hybrid class but were continuing with “traditional.”

    ASHI

Sounds like we are going to switch. Doesn't seem like a huge change, I'll probably keep my AHA instructor for private classes.

    ASHI

Does anyone here instruct under ASHI? We are debating switching from AHA for BLS and ACLS and I have no idea if this is a good idea or not. I have been an AHA instructor for a long time and while I hate teaching it, I have a good understanding of the content and whip through it quick. I guess...

We get patients assigned to the waiting room when we patch. Holding the wall is not a thing here and I ain’t mad about it.

It is now, but the coordinators don’t have any rank. To me it’s important we have a chief officer who actually knows how to run an ambulance service and can make timely, command decisions. The coordinators do not have that authority, nor should they really. We used to have single role medics but...

    Best ems vent right now?

The Zoll Z Vent is excellent. Much less featured than a T1 but if you aren't doing CCT where you are trying to match complex vent settings I think it works very well. Comes with a nice case as well.

Our EMS chief has so been ineffective that the Fire Chief is eliminating the position entirely. EMS is now a “responsibility” of the non paramedic training chief. Our EMS coordinator and educator will work for him and that’s it. We’re making a great example of why everyone thinks Fire based...

    ALS/BLS Handover

Nope. We no longer really have specific policies directing assessments, I think a good onboarding and then later QI is a better tool to address deficiencies.

    ALS/BLS Handover

I dunno. If a paramedic does an adequate assessment with nothing found, what’s wrong with having EMTs attend? The same thing happens in the ED with different provider levels and the patient is still going to the ED in the ambulance. I encourage the paramedics to provide the most thorough...

    ALS/BLS Handover

We run Paramedic/EMT staffed ambulances and do not use specific criteria for which provider attends. It is expected that the paramedic observe the EMT's assessment if the EMT is leading the call. Paramedics can add their own assessments in or do it entirely themselves and then downgrade the...
