Have never had a problem with either of them. Maybe its operator error?

We had that problem last winter. The end of the glass syringe wasn't long enough to activate the plunger in the lock or tubing connection. We have since bought some adapters for them and they are kept with the adenosine. An alternative is to draw up the adenosine into an empty 5 or 10cc syringe as you would a drug from an ampoule and then give it.
